Collegiate’s Back to School website is now live and has been reorganized to make it easier for parents to find the information and resources needed to start the 2012-13 school year successfully. PLUS, access to your online Student Information and Health Update form will be sent via email. Read on for important information….

UPDATE YOUR CHILD’S INFORMATION AND ORDER SUPPLIES ONLINE

Placing Back to School information and the form online saves paper and time spent copying and re-keying the data. Here’s how it works:

Visit the Back to School website for information on books and supplies, downloadable medical and physical forms, athletics, lunch plans and detailed information for students in every grade level.

An email was sent on July 25 to one parent in each household for every child who attends Collegiate. That parent can click on the secure link to the Student Information and Health Update form and fill it out, or forward the email and link to the other parent to complete. The process is easier to navigate than in previous years, and some of the information on the form will be pre-populated.

The Student Information and Health Update form is due by July 30 for fall athletics and Aug. 10 for all other 7th-12th Graders. Your child will not be allowed to participate in practices until we have received all relevant health form updates.

NOTE: You must complete the Student Information and Health Update form in one sitting, because it cannot be saved. Be sure you have all of the information you need handy before you begin the form.

Once you submit the form, you’ll be directed to the Back to School website.

“Ultimately, we didn't want to ask parents to fill out the same information again and again on multiple paper forms,” says Patty Sinkler, Associate Director of Technology. “Everything is all in one place and easy to find. We are trying to make it as easy as possible for parents.”
